You have a few options for submitting an application for review:
Online Submission:
- Note: The online application cannot be saved once initiated, and it times out after one hour of open access to a section. In case of timing out, your data won't be saved or sent to the Office of Industrial Relations.
Print and Send:
- Print the Application for Claim Review form (PDF, 0.2 MB) or the Application for a Premium/Policy Review form (PDF, 0.18 MB).
- Submit the printed form via fax, email, or mail to the Review Unit. Ensure all sections are completed, provide grounds for review, attach supporting documents, and sign the form.
Request a Copy:
- If you prefer to receive a copy of the application for review via mail or email, get in touch with the Review Unit to make the request.
